Powered wheelchairs & batteries
The part that goes wrong. Battery chemistry and watt-hours decide whether your chair flies, and the rules differ depending on whether the battery can be removed.
Lithium-powered chairs are accepted only if the chair is foldable and the batteries are easily removable; VRLA/AGM gel chairs must be switched off and are checked only, with a maximum of 12 V; spillable batteries are refused. On flights operated by Azul Conecta, customers who need a wheelchair to get around are not accepted at all.

"Se a sua cadeira de rodas ou equipamento de mobilidade for movido a bateria de Íon Lithium ou Metal Lithium, ela deve ser dobrável e as baterias facilmente removidas, para permitir essa dobra" — accepted only if the chair folds and the battery comes out easily.

"A potência deve ser de: uma bateria de até 300Wh ou duas baterias de até 160Wh cada. Em ambos os casos, as baterias devem estar instaladas na cadeira de rodas, que só é permitida como bagagem despachada." NOTE: this directly contradicts the preceding sentence on the same page, which says the batteries must be removed and carried in hand luggage.

Azul publishes ONE cap and applies it to both scenarios. On the baggage page it follows the removal rule: "as baterias são levadas na bagagem de mão, de forma isolada… É necessário também que seja uma bateria de até 300Wh ou duas baterias de até 160Wh cada" — the very next sentence then says the batteries "precisam estar instaladas na cadeira de rodas".

Azul does not publish a rule for spare wheelchair batteries; the 300 Wh / 2 x 160 Wh figures describe the batteries that power the chair.
Azul does not publish how many spare wheelchair batteries may be carried.
For lithium-ion / lithium-metal chairs: "as baterias [devem ser] facilmente removidas… Nesses casos, você deverá levá-las na bagagem de mão, de forma isolada." Azul also says removable batteries travel as hand luggage while the chair is checked separately; non-removable ones are checked with the chair.

"Cadeira de rodas ou equipamento de mobilidade movido a bateria não derramável com tecnologia VRLA ou AGM (gel), dobrável ou não, deve estar desligada para evitar curto-circuito na aeronave e só é permitida como bagagem despachada. Além disso, a potência máxima deve ser de 12V."

Azul does not publish a battery spec sheet, MSDS or form requirement for wheelchair batteries.
Azul publishes no maximum dimensions for a powered wheelchair.
Azul publishes no weight limit for a powered wheelchair.
Azul does not publish whether a powered chair is returned at the aircraft door.
Azul cites Article 48 of Ordinance (Portaria) 676 of the Brazilian Civil Aviation Department: gel, AGM and lithium batteries are authorised, with non-dismountable batteries checked together with the chair and dismountable batteries carried as hand luggage while the chair is checked separately.
Manual wheelchairs
ROUTE EXCLUSION: on flights operated by Azul Conecta (its Cessna Caravan regional arm) customers who need a wheelchair to get around, or who need special ascent/descent equipment, are not accepted at all, and no boarding or disembarking assistance is provided — check the operating carrier before booking.

"Também é possível transportar com você a bordo, se preferir" (you may also carry it with you on board, if you prefer). Azul publishes no cabin stowage dimensions and states elsewhere that battery-powered chairs are only allowed as checked baggage, so this appears to apply to an unpowered folding chair.

Azul publishes no maximum wheelchair dimensions.
Azul publishes no weight limit for a checked wheelchair.

Booking & cost
Also 72h if a companion is required; FREMEC between 30 calendar days and 7 working days. No stricter powered-chair deadline is published.

"Ao comprar o seu voo pelo site ou app, clique em 'Atendimento Especial'… Você também pode solicitar assistência após a compra, entrando em contato com Azul Center: 4003 1118 ou 0800 887 1118." The wheelchair service can also be requested at check-in or at the airport.

Not published. "O despacho de um item de acessibilidade, como cadeira de rodas ou muletas, é gratuito" uses the indefinite article — Azul's own English page renders it "Checking in AN accessibility item… is free" — and the baggage page says "sua cadeira de rodas OU OUTRO equipamento que precisa para sua locomoção". No number is published.
Not published for mobility aids. The checked-baggage page states only a general goods limit — "O valor máximo é 1.131 DES… que é o limite de indenização" — framed as a ceiling on the declared value of baggage Azul will carry at all, not as a wheelchair damage-liability rule, so it is not recorded here.
On board
Azul does not publish whether an on-board aisle chair is available.
Not published on Azul's site.
Boarding and disembarking assistance is provided on Azul mainline flights, but "em voos operados pela Azul Conecta, a assistência em embarque e desembarque não está disponível" and customers who need a wheelchair to get around, or who need special ascent/descent equipment, are not accepted on those flights.

Azul publishes no movable-armrest or bulkhead seating policy for wheelchair users (it only excludes emergency-exit rows for pregnant travellers).
Fleet detail. Azul Conecta (its Cessna Caravan regional operation) does not accept customers who use a wheelchair for locomotion or who need special ascent/descent equipment, and provides no boarding or disembarking assistance.
Worth knowing
Read from Azul's Portuguese pages and cross-checked against its own English translation, which is near-literal but diverges in one material place. THE BIG THING A HUMAN MUST RE-CHECK: within a single paragraph Azul says lithium batteries must be removed and carried in hand luggage AND that "in both cases the batteries must be installed in the wheelchair, which is only allowed as checked baggage". Both readings are recorded (installed and removed caps are the same single published 300 Wh / 2 x 160 Wh figure). Second: Azul Conecta-operated flights refuse customers who need a wheelchair to get around — a hard route exclusion a traveller must check before booking. Third: the Portuguese page says Azul's medical department answers companion requests "no prazo de até 7 (sete) dias úteis" while the English page says "within 48 hours"; the English page is the outlier. Azul publishes no wheelchair size or weight limit, no count of free mobility aids (both language versions use an indefinite article, not "one"), no on-board aisle chair statement and no mobility-aid damage-liability policy — the 1,131 SDR figure on the baggage page caps the declared value of checked goods generally and is not a wheelchair liability rule.
